Participant Registration & Policy Forms

In order to complete your registration, you must digitally complete and sign the forms below.

Form 1 and Form 2 are required for all participants and are combined into one packet. Form 3 is required for all minor participants (even if accompanied by parents). Form 4 is required for every chaperone/guardian of a minor participant (even if said chaperone is a parent).

For all participants

• Form 1 and Form 2: Waiver and Release of Liability (1) and Dress Code Policy (2)

For minor participants

• Form 3: Medical Authorization & Emergency Treatment Consent for Minors

For designated chaperones (including parents) of minor participants

• Form 4: Chaperone Acknowledgment and Responsibility Agreement
